Click the Volunteer Application Form button.

Click the Download button in the upper right corner of the form.

Save the form on your computer.

Open the form.

Complete the form. There is red RESET button at the top right of the form if you want to start over.

After you have completed the form, save the form to your computer.

Review the application checklist at the bottom of the form.

At the bottom of the completed form (saved on your computer) click the green SUBMIT button.
